Summary Provides personal care and housekeeping/companion services to clients in the home or other environments where the client resides in accordance with an established individualized plan of care and under the supervision of a Registered Nurse (RN). Provides care and comfort to clients in accordance with assigned tasks and the established plan of care. Observes the client, reports and documents observations and care performed. Provides timely documentation of services based on the clients care plan. Essential Duties and Responsibilities include the following. Other duties may be assigned. Personal Care Services Ensure the quality and safe delivery of personal care services to the client while also supporting his/her families and caregivers.Perform or assist client with personal care activities as prescribed in the care plan, including, but not limited to, hygiene, mobility and elimination. If unable to perform a certain task, reports to the Assistant Director/ Clinical Scheduler immediately. .Provide and maintain safe environment for client by using equipment safely and properly such as side rails, oxygen, etc.Practices universal precautions in all contacts with clients.Perform support activities as prescribed in the care plan including but not limited to: supporting independence, providing companionship and providing respite for families.Respect the autonomy and right to confidentiality of all clients and families.Monitor skin condition and basic elements and changes in body functioning and record/reports appropriately.Practice safe transfer, ambulation, normal range of motion and positioning techniques.Identifies changes in client conditions and reports to Assistant Director.Document care provided and response of client/family to care and submit documentation in a timely manner in accordance with agency policies.Work in cooperation with family/caregiver to meet the emotional needs of the client/family/caregiver.Perform or assist client/family with home maintenance activities as prescribed in the care plan including, but not limited to light housekeeping and meal preparation.Monitor adequate fluid and nutrition intake. Assist in adherence to prescribed diet.Provide transportation and/ or complete errands as prescribed in care plan and adhering to agency policy.Assist in medication compliance by providing medication reminders as outlined in care plan
