Psychiatric Mental Health Nurse Practitioner (CTRI) (Healthcare)



The Community Translational Research Institute  (CTRI)  CTRI brings together  key  institutions and individuals from the public and private sectors, including academic, public health and community medicine institutions, as well as neighborhood schools and community-based organizations for the prevention of chronic diseases. CTRI transforms population health practice through  translation  of  prevention science into community-based programs and sustained practice and policy. The founding CTRI partners include leaders of the Claremont Graduate University School of Community and Global Health, the County of Riverside, the Inland Empire Health Plan, and the University of California Riverside School of Medicine.

Heluna Health invites applicants to apply for the position of Psychiatric Mental Health Nurse Practitioner. Works with the Enhanced Care Management (ECM) and Community Support teams to (summarize duties) manage caseloads with complex medical and/or psychiatric needs,complete Comprehensive Health Assessments( CHAs)  and medication reconciliation. The Nurse will also facilitate health goal-setting and create Shared Care Plans.

This is a full time (40 hours per week), benefited position. Employment is provided by Heluna Health

The pay rate for this role is up to $86.53/hour, depending on experience and qualifications.

ESSENTIAL FUNCTIONS

  • Manage caseloads of members with complex medical and/or psychiatric needs (Tiers 1 & 2)
  • Complete Comprehensive Health Assessments (CHAs) and facilitate thorough medication reconciliation to support safe and effective care.
  • Facilitate individualized health goal-setting and develop Shared Care Plans that align with member priorities and ECM milestones
  • Coordinate care with Primary Care Providers (PCPs), behavioral health specialists, and community resources to support whole-person care.
  • Track member progress and outcomes using the Electronic Health Record (EHR) system and contribute to quality improvement efforts.
  • Lead care transitions and help ensure continuity of care through proactive follow-up and planning
  • Proactively flag medication alerts, safety concerns, or care gaps and engage the care team in resolution.
  • Provide psychiatric evaluations, medication management, and crisis intervention when needed.
  • Support ongoing team training and serve as a clinical resource to care managers and staff.
  • Participate in interdisciplinary team meetings, case reviews, and ECM program reporting.
  • Participate in case reviews, case conferences, and co-review Comprehensive Health Assessments (CHAs) alongside the ECM Care team to inform shared care planning.
  • Other duties assigned.

JOB QUALIFICATIONS

Education/Experience

  • Required active PMNHP certification.
  • A Master's degree or higher from an accredited college or university in a discipline related to the core business function of the department (Health Information Systems, Public Health, Public Policy, Psychology, Social Work, etc), preferred.
  • Experience with Case Management, certification of Case Management preferred.
  • Experience with researching, studying, and making recommendations to support health or social service programs or policy.
  • Four (4) years in a highly responsible management experience in program administration for underserved populations preferred.
  • Strong organizational skills, including an ability to manage multiple work projects simultaneously, track project details, and meet deadlines.
  • Strong technical skills with Microsoft excel and experience with database management (e.g., Electronic Health Record Systems) preferred.
  • Ability to attend meetings, provide training, technical assistance,  and  other job-related  duties in locations throughout Southern California and have reliable transportation to carry out essential functions.

Certificates/Licenses/Clearances

  • Active certification as a Psychiatric-Mental Health Nurse Practitioner
  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

  • Proficient skill set in using an array of Microsoft Office Suite software programs such as Word, Excel, PowerPoint, Access, Adobe Reader, One Note, Outlook, Publisher, Teams, Outlook, Zoom etc.
  • Able to multi-task and set workload priorities for time sensitive projects/tasks.
  • Ability  to  problem  solve and  make recommendations to processes, policies, etc.
  • Able to communicate with all levels of personnel, e.g., written, verbal, in a professional and concise/clear manner; ability to work within a project team and/or independently.
  • Able to work in a very diverse environment and with diverse individuals.
  • Ability to be flexible in meeting changing work tasks and timelines; must be dependable and reliable.
