DC Licensed Family Nurse Practitioner - DC Hope Center (Healthcare)

JOB SUMMARY

This position is a member of the medical team and supports the medical care and treatment of the persons served. Under the direction of the Vice President of MD Programs and the Medical Director, the Family Nurse Practitioner will provide effective, efficient and appropriate primary care health services, patients health screening and other health review assessments in order to serve the community's healthcare needs.

RESPONSIBILITIES

  • Assists Medical Director in developing clinical practice guidelines
  • Participates in provider and medical care team meetings
  • Establishes positive relationships with local medical community
  • Interviews patients to obtain history, performs physical examinations, orders lab and other tests, prescribes medications and treatments
  • Provides continuity of care for patients
  • Makes referrals for secondary and tertiary care
  • Performs medical procedures according to privileges issued
  • Provides health maintenance visits, evaluates for immunizations and provides anticipatory guidance and referrals
  • Documents and codes all patient contact accurately and legibly in the medical record
  • Skill in providing excellent customer service and support; organizing and prioritizing workload and meeting deadlines; and excellent written and verbal communication
  • Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial, ethnic and professional backgrounds
  • Prescribes medication as part of a treatment plan and conducts regular follow ups.

EXPECTATIONS

  • Participate a physician member of a multi-disciplinary patient care team.
  • Complete all billable notes within 24 hours of service rendered.
  • Participate in peer review process as needed
  • Provide consultation to behavioral health team as needed.
  • Evaluates and reviews the work of other medical providers for appropriate diagnosis, treatment and chart documentation under peer review.
  • Work effectively with departmental staff, private physicians, clients and others contracted in the course of work.
  • Community collaboration with key health stakeholders, other divisions of the Department, and other County departments.

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.
  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment. ­
  • Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
  • Operating office equipment requiring continuous or repetitive hand/arm movements.
  • The ability to remain in a sitting position for extended periods of time.

  • Becomes an internal subject matter expert on patient centered care
  • Maintain effective relationships with other County Departments, County policy makers and community health stakeholders
  • Conduct in-service training programs as needed.
  • Creates and maintains an environment of care for persons served and becomes a value member of the community.
  • Participate in the analysis, evaluation and planning of health or mental health programs.
  • Review appropriate paperwork for documentation of clinical services and medical assessments.

REQUIREMENTS

  • Current Licensed as a Family Nurse Practitioner in the State of Maryland.
  • Minimum two (2) to four (4) years of experience practicing primary care within a community health center (FQHC) or Clinic serving individuals with mental illness, behavioral health concerns or related co-occurring health conditions such as substance abuse or chronic health conditions.
  • Graduate of an accredited Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners and/or a degree in nursing or related field a plus. Degrees from outside of the United States must be reviewed and accepted by the World Educational Services and validated.
  • Must have a current DEA certification.
  • Must have CPR/First Aide certification.
  • Trained in cultural competency and working with diverse populations.
  • Must have experience with electronic medical records systems foothold/awards
  • Excellent computer skills in Microsoft Office Suites.
  • Excellent written and verbal communication skills.
  • Bilingual in any language a plus.
